Phase 4 leverages the mature Roadmap Execution Steering Committee, PPP, and Coordinating Center to continue enhancing the AHRQ DKP-supported knowledge ecosystem and other outputs from the previous phases to ensure that the ACTS Future Vision (as further refined and by widely adopted based on PPP efforts) is achieved to the fullest and broadest extent possible. This includes significant, measurable advances in realizing Healthy People 2030 (115) goals and other key objectives for improving health and care delivery. During this phase, the Roadmap Execution Steering Committee, PPP, and Coordinating Center will also establish an ACTS Roadmap for the next decade.

Phase 4 Deliverables

  • Mature PPP-driven collaboration network and infrastructure and Coordinating Center
  • Maintenance of a Production version, enterprise-wide AHRQ DKP
  • Enhanced Production version of tooling that makes information processing in the knowledge ecosystem cycle computable and highly valuable in supporting LHSs and the broad achievement of the Quintuple Aim for all high-priority clinical targets and includes a full Production version of an open, public-sector marketplace that interoperates smoothly with interested private marketplaces
  • Living evidence, guidance, and CDS interventions/eCQMs for the 25 clinical targets prioritized by the Roadmap Execution Steering Committee/PPP produced and maintained using the tooling deliverables
  • Enhanced playbook for creating and maintaining living, computable evidence, guidance, and CDS/eCQMs at least 25 different Phase 3 clinical targets into at least 300 different CDOs total (various sizes, populations served, geographies) that coordinate with investments from other stakeholder organizations to generate supporting educational materials (e.g., courses, learning communities)
  • 50 different CDOs (with variation in size, populations and regions served, health IT sophistication and systems) demonstrating LHS function (e.g., continuous improvement in targeted outcomes by successfully leveraging a high-functioning knowledge ecosystem supported by a leadership-led learning culture)
  • Enhanced Playbook for successfully implementing CDS interventions and eCQMs covering in detail at least the 25 PPP-prioritized clinical targets. that is coordinated with investments from other stakeholder organizations to generate supporting educational materials (e.g., courses, learning communities)
  • A data-driven, summative evaluation of the Roadmap execution (emphasizing Phase 4 results) that may include user surveys and evaluations and analyses of health outcomes compared to targets, which will yield the progress, conclusions, and recommendations for improvement that provide the basis for the ACTS 2.0 enhancement and expansion stakeholder-driven Roadmap for the next decade
  • Requirements and project plans for stakeholder-driven enhancements to the AHRQ DKP and other knowledge ecosystem components to be executed over a subsequent 10-year ACTS Roadmap 2.0 horizon
